As great as the main game is I severely struggle with being positive about the DLC. So much so in fact, that I wouldn't recommend getting it and this is only for one very menial little reason.
Saboteurs.
Every now and again, the game will grind to a halt and force you to play Where's Waldo. Where you have to scour your hospital in search for a Saboteur. They look like a mechanical variant of one of the alien races and once you found them you need to destroy them with two clicks. First you gotta click the saboteur, next you gotta click the destroy button.
The basegame had clickable units too. There were those that would reward you with resources. And small little pests that ran into your hospital. But you could either have enough staff to deal with them, or place traps, or you had to click them. Even while these little pests looked massively different from all the NPC's you were able to automate dealing with them. Which means you can progress towards the point where you no longer need to deal with them. Before that time, you could just quickly click them. A single click, and splat, dead. Easy. Fast. The Saboteurs are not automate able and require two clicks per saboteur. Very annoying.
There already was a nuisance in the game called Asteroids. No way of dealing with those. They just happen, they disable parts of your hospital, and it would close rooms for a while.
One of those is enough. Saboteurs is entirely too much effort for me to care about it.
Pass on the DLC. Definitely play the basegame though.
